Issue of Duplicate Marksheet / Diploma Certificate |

Duplicate Mark Sheet/Diploma certificate will be issued upon production of the following |

| |
• Request letter along with a copy of any credential in support of course studied or a xerox copy of mark sheet/diploma certificate, if possible. |
|
|
| |
| |
|
| |
• Demand Draft for Rs.100/- for duplicate mark sheet and Rs.200/- for duplicate Diploma certificate to be drawn in favour of Director General, CIPET-Corporate A/c No. 10565622390 payable at Chennai. |
| |
|
| |
• If required to be sent by post, a self addressed envelope with Rs.60/- value of stamp affixed, need to be enclosed. |
| |
|
| |
• Declaration in a non-judicial stamp paper of value of Rs.10/- in the prescribed format. |
| |
|
| |
• A copy of FIR Report filed. |
| |
|
| |
|
| |
• Duplicate certificate will be issued within 10 days from the date of receipt of request along with all requisite documents. |
